Job summary

MedCap Health is hiring a family, geriatric, or adult Nurse Practitioner for post-acute and long-term care settings. The role offers full-time, part-time, and flexible scheduling with supportive administrative resources.

You will provide autonomous management of complex chronic diseases, work closely with attending physicians, and participate in a multidisciplinary team to optimize patient outcomes. No overnight requirements are specified, and malpractice coverage is included.

Qualifications

• Board-certified NP with focus on geriatrics/post-acute care.
• 1+ year of experience in post-acute/SNF/LTC settings.
• Prior post-acute, long-term, and/or skilled nursing facility experience preferred.

Responsibilities

• Provide high-quality management for patients with chronic diseases in post-acute and long-term care settings.
• Collaborate with attending physicians, nursing leadership, and social work to coordinate comprehensive care.
• Deliver patient-centered care plans and adjustments in collaboration with the care team.
