The Post-Acute Liaison (Occupational or Physical Therapist) for Inpatient Rehabilitation plays a vital role in ensuring patients transition to the appropriate inpatient rehab facility at the right time. This position involves collaborating with other post-acute liaisons, processing referrals from acute care, and reviewing patient cases for clinical eligibility. The goal is to build trusting relationships with case managers and providers to place patients, such as those with brain injuries, orthopedic conditions, or loss of function, in the optimal care setting for recovery.

What You Can Expect

  • Reviewing referrals for appropriate placement
  • Foster the relationship between patients and the facilities providing their care
  • Meeting with patients to describe our hospital and how the services we provide will benefit them
  • Working closely with providers and case managers for possible referrals.
  • Schedule: Monday-Friday, some evening and weekend coverage.

Qualifications

  • Education: Associate's degree or higher
  • Experience: 1 year relevant experience
  • Licenses/Certifications:
    • Valid license to practice physical therapy or occupational therapy in state of practice
    • Valid Driver's license and vehicle insurance,
    • Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
  • Preferred Qualifications:
      • Occupational or Physical Therapist
      • Liaison experience
      • Post acute care experience
