Job Overview:

As part of RWJ Barnabas Health Corporate Population Health team, you will play an instrumental role in a department that provides fully coordinated patient care from a holistic approach to improve the overall care of patients involved in value-based, ACO, and bundled payment programs across the healthcare continuum. The Post-Acute Care Coordinator (LPN, RN, PT, OT, or SW) evaluates treatment plans and goals at Post-Acute Care facilities. The coordinator partners with clinical teams to ensure patient progress towards discharge goals, coordinates discharge planning, and facilitates follow-up with providers and community services. Additionally, the coordinator assists patients in transitioning to the next site of care.

Qualifications:

Required:

  • Degree requirements vary by role: LPN (Associate's), RN (Associate's), Physical Therapist (Bachelor's), Occupational Therapist (Bachelor's)
  • 3-5 years experience managing relationships between acute and post-acute facilities
  • Ability to travel to post-acute and hospital sites
  • Strong organizational and leadership skills

Preferred:

  • Higher degrees (Bachelor's or Master's) in relevant fields
  • Experience in Post-Acute Care Settings
  • Knowledge of Value-Based Programs

Licenses Required:

  • NJ LPN, RN, Social Worker, Physical Therapist, or Occupational Therapist License

Scheduling:

  • Monday-Friday, 9:00 AM - 5:00 PM
  • Support for Middlesex, Hudson, and Somerset Counties; location may change based on business needs

Essential Functions:

  • Assess patient condition during and after post-acute episodes
  • Develop and evaluate care plans involving patients and families
  • Participate in multidisciplinary rounds to create collaborative, evidence-based care plans
  • Coordinate discharge planning and follow-up care
  • Educate staff and patients, and maintain detailed documentation
  • Provide resources and education on readmissions to post-acute facilities
  • Serve as a resource for post-acute care facilities
