Job Overview:

As part of RWJ Barnabas Health Corporate Population Health team, you will play an instrumental role in a department that provides fully coordinated patient care from a holistic approach, aiming to improve the overall care of patients who are part of the value-based, ACO, and bundled payment programs across the healthcare continuum. The Post-Acute Care Coordinator (LPN, RN, PT, OT, or SW) evaluates treatment plans and goals of care at Post-Acute Care facilities. They partner with clinical teams to ensure patients are progressing towards discharge goals, coordinate discharge planning, and facilitate timely follow-up with providers and community services. They also assist patients transitioning to the next site of care.

Qualifications:

Required:

  • LPN - Associate's Degree; RN - Associate's Degree; Physical Therapist - Bachelor's Degree; Occupational Therapist - Bachelor's Degree
  • 3-5 years experience managing relationships between acute and post-acute facilities
  • Ability to travel to post-acute and hospital sites
  • Strong organizational and leadership skills

Preferred:

  • RN - Bachelor's Degree; Physical Therapist - Master's Degree; Occupational Therapist - Master's Degree
  • Experience in Post-Acute Care Settings
  • Knowledge of Value-Based Programs

Licenses Required:

  • NJ LPN, RN, Social Worker, Physical Therapist, or Occupational Therapist License

Scheduling Requirements:

  • Monday to Friday, 9:00 AM - 5:00 PM
  • Support for Middlesex, Hudson, and Somerset Counties (subject to change)

Essential Functions:

  • Assess patient's condition during and after post-acute episodes
  • Develop and evaluate care plans involving patients and families
  • Participate in multidisciplinary rounds to create collaborative care plans
  • Coordinate discharge planning and follow-up care
  • Provide education to staff and patients
  • Maintain detailed documentation
  • Support post-acute care facilities on readmission prevention
  • Serve as a resource for facilities

Other Duties: Responsibilities may change as needed.
