CHRISTUS Health is seeking a Post Acute Care Coordinator to support the Population Health Care Management Department. The role focuses on coordinating post-acute care, facilitating communication among clinicians, RN Navigators, and care teams, and ensuring seamless transitions through EMR documentation and outreach.

The position emphasizes strong organizational and oral communication skills, with routine documentation, reporting, and collaboration in a fast-paced healthcare setting.

Qualifications

• 2 years in a healthcare setting
• Experience with post-acute preferred
• Experience with EPIC or other EHRs
• Proficient with Microsoft Office applications

Responsibilities

• Facilitate communication between Agencies, Clinicians, RN Navigators, Care Coordinators and leadership.
• Coordinate post-acute care transitions and monitor patients’ care plan progress.
• Import and organize clinical documentation into the EMR for timely care.
• Conduct outreach to facilities to confirm discharge dates and Plan of Care status.
• Maintain records and generate operational reports (productivity, utilization).
