Ensures timely completion of precertification and authorization activities to facilitate smooth patient transfers and reduce delays. Collaborates with case management, social work, and clinical teams to maintain accurate documentation and communication with post-acute providers. Applies basic knowledge of administrative and healthcare processes to support efficient patient throughput and continuity of care. ## Essential Functions• Initiates and completes precertification and authorization requests for post-acute care services in accordance with payer requirements.• Communicates with insurance companies, post-acute facilities, and internal departments to coordinate discharge arrangements.• Assists case managers and social workers in facilitating transitions to lower levels of care, ensuring all required documentation is accurate and complete.• Tracks and updates patient authorization status in electronic health record systems to support timely discharge.• Verifies insurance eligibility, benefits, and coverage for post-acute services and communicates findings to appropriate team members.• Maintains organized records of authorizations, correspondence, and approvals for compliance and reporting purposes.• Responds promptly to inquiries from patients, families, and care teams regarding post-acute placement and insurance processes.• Follows established protocols to escalate delays, denials, or complex cases to supervisory staff for resolution.• Adheres to confidentiality, quality, and compliance standards in handling patient and payer information.• Performs other duties as assigned. ## Education RequirementHigh school diploma or equivalent experience- ## Experience Requirement0-2 years of experience in a healthcare, clinical setting, or a relevant area An equivalent combination of education and experience may be considered. All experience must be paid and in the same related field. Part-time and PRN experience will be prorated based on hours worked per week.
