A leading company seeks a Life Enrichment & Social Services Coordinator to enhance the well-being of residents through engaging activities and coordinated services. Position Summary:
The Life Enrichment & Social Services Coordinator plays a dual and integrated role in promoting the emotional, cognitive, social, and psychological well-being of residents at Portola Gardens through its programs and the coordination with outside service providers. This position is responsible for developing and implementing a vibrant calendar of person-centered activities while also helping to coordinate eligibility and access to onsite and external insurance based clinical/psycho-social services including mental health support and care coordination.

Key Responsibilities:
I. Life Enrichment Programming
Develop and execute a monthly calendar of meaningful, engaging, and diverse activities that support residents' cognitive, physical, emotional, and social well-being.
Lead or coordinate room visits, group activities, events, family engagement, holiday celebrations.
Accountable for meeting the 7 dimensions of wellness standards
Support new resident move-ins and transitions at the community
Customize programming for residents with mental health, memory loss, mobility impairments, or social withdrawal.
Collaborate with other departments (e.g., Food service, Clinical, Housekeeping ) to support event logistics and resident participation.
Maintain accurate participation records and evaluate resident engagement and satisfaction.
Manage part-time life enrichment coordinators
Recruit and manage volunteers, entertainers, and outside program partners.
Conduct Complete the Getting to Know you Activity Survey and at regular intervals as conditions change

II. Social Services Coordination
Work with Keystone, PG Move-in Coordinator and Wellness Department to maintain updated eligibility data for residents receiving or requesting outside insurance-based programs. Manage the regular updates with residents, insurance plans and health care providers.
Coordinate intern services and schedules to help ensure continuity of care and services between Keystone and PG
Assist Keystone with obtaining current services list and provide billing information to appropriate team members
Schedule interdisciplinary care conferences as requested
Support residents and families in navigating emotional or behavioral health concerns, grief, adjustment to communal living, or conflict resolution.
In conjunction with the Wellness Department coordinate referrals to Keystone and other outside mental health providers.
Assist with applications and renewals for public benefits or programs (e.g., Medicaid, SSI, VA benefits, ALWP, CalAIM).
In conjunction with Wellness Department, support communication and paperwork regarding discharge planning, transitions to higher care levels, and family meetings.
In conjunction with the Wellness Department Proactively communicate changes in residents' psychosocial, behavioral, or mental health status to all relevant stakeholders, including internal care staff, clinical team members, outside service providers, and families/responsibility parties
Maintain confidential resident case notes and documentation in compliance with HIPAA.
Qualifications:
Bachelor's degree in social work, Gerontology, Psychology, Therapeutic Recreation, or a related field
At least 2 years of experience working with older adults in a long-term care, assisted living, or social services setting.
Strong understanding of aging-related mental health, dementia, and eligibility-based benefit programs.
Proven ability to plan and lead recreational and wellness programs tailored to older adults.
Excellent interpersonal, communication, and problem-solving skills.
Compassionate demeanor and ability to build trust with residents and families.
Proficient in basic computer applications (Microsoft Office, EHR/EMR systems preferred).
Preferred Competencies:
Knowledge of trauma-informed care and behavioral intervention strategies
Experience with person-centered planning
Familiarity with third party payers MediCal waiver programs and CalAIM a plus
Need to be open to having flexible schedule, weekend and evening work, some on call work
Salary range 67,000-75,000
