Job summary

Cedar County Memorial Hospital is seeking a Population Health Nurse Assistant to support the QAPI program through data management, chart reviews, and population health initiatives. You will coordinate outreach, monitor quality measures, and help identify care gaps under the direction of the QAPI RN.

The role emphasizes non-clinical, population health duties with strict adherence to policies, HIPAA, and regulatory requirements. Collaboration with interdisciplinary teams is essential.

Qualifications

• High school diploma or equivalent.
• Minimum of 2 years of healthcare experience in hospital, clinic, or population health settings.
• Healthcare-related certification or training; CNA preferred/required as determined by the hospital.
• Current BLS certification or ability to obtain.
• Demonstrated experience with electronic health records and data management.
• Strong organizational, analytical, communication, and interpersonal skills.
• Ability to manage multiple assignments and meet deadlines.
• Ability to work independently within established guidelines.

Responsibilities

• Support QAPI RN in development, implementation, and monitoring of quality initiatives.
• Independently complete chart reviews, audits, data collection, and quality tracking.
• Identify and track patient care gaps and preventive care needs.
• Conduct patient outreach and coordinate follow-up with QAPI RN.
• Maintain population health registries, reports, and electronic records.
• Identify barriers to patient care and connect patients to resources.
• Escalate clinical concerns or safety issues to the QAPI RN.
• Maintain HIPAA compliance and hospital policies.
• Participate in community and population health initiatives as assigned.
• Perform other QAPI and population health duties as needed.
