Trillium Health Resources is a Tailored Plan and Managed Care Organization (MCO) that manages serious mental health, substance use, traumatic brain injury, and intellectual/developmental disability services in North Carolina. Serving in 46 counties, we help individuals and their families strengthen well-being and build foundations for a healthy life.

Join our team as we empower others to live their best lives by providing access to quality healthcare. We offer a challenging, engaging work environment where staff take home more than a paycheck. Every day, we see the results of our dedication – in the smiles of children on our accessible playgrounds and in the pride on the face of an adult cooking a meal for the first time. Working at Trillium Health Resources is more than just a job; it is an opportunity to make a direct impact on the communities we serve.

At Trillium, we know that empowering others begins with supporting and developing our team. That’s why we offer competitive benefits and work-from-home flexibility so that our employees thrive outside of the office. We’re also committed to building a diverse, inclusive culture where all employees have the potential to grow professionally and personally.

What We’re Looking For

Trillium Health Resources is seeking a dedicated and passionate Population Health Specialist to join our team and make a tangible impact on the lives of individuals within our community. This vital role is at the forefront of our commitment to improved health outcomes and is designed to be a dynamic and rewarding opportunity for the right candidate. If you are a compassionate, resourceful, and driven individual with a passion for community health, we encourage you to apply. On a typical day, you might:

  • Assist with the development and coordination of projects.
  • Outreach to support care gap closures and other organizational identified engagement initiatives.
  • Perform supportive outreach, follow-up, and engagement with members.
  • Facilitate programs and activities that promote holistic health and improve members’ health literacy/education (teaching self-management skills, sharing self-help recovery resources, educating on chronic conditions, etc.).
  • Ensure the proper storage and transmission of sensitive health records.
  • Provide any additional support services/referrals in addressing unmet health-related resource needs, such as housing, transportation, nutritional food sites, and other services as needed.
  • Document activities in the Trillium Connections care management platform. Documentation should be timely, accurate, complete, and reflective of the member’s needs.
  • Review member information within the platform, update information as needed, and/or inform the appropriate staff when inaccuracies are found.
  • Develop and cultivate working relationships with service providers and community other community partners.
