Job summary

CommunityCare HMO Inc. in Oklahoma seeks a Care Manager to lead care coordination and population health initiatives for members with mental health needs.

You will assess psychosocial needs, develop person-centered care plans, and coordinate referrals to behavioral health and community resources, while ensuring compliance with HIPAA and CMS standards. Four+ years of clinical mental health experience and active Oklahoma licenses are required.

Qualifications

• Master's degree in a mental health field.
• Active, unrestricted licenses as social worker and mental health professional in Oklahoma.
• Minimum of four years related clinical experience in mental health.
• Managed behavioral health experience preferred.
• Ability to diagnose mental health cases and refer to appropriate levels of care.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify member needs and social determinants of health.
• Develop individualized, person-centered care plans with members and care team.
• Coordinate referrals to behavioral health services and community resources.
• Support transitions of care after hospitalizations and other care events.
• Monitor progress and maintain documentation in designated systems.
• Ensure compliance with HIPAA, CMS requirements and URAC accreditation standards.
• Participate in quality improvement initiatives.
