Responsibilities

• Lead planning, implementation, and evaluation of population health initiatives.
• Coordinate cross-functional projects with clinical, operational, and community partners.
• Develop project plans, timelines, workflows, and reporting processes.
• Monitor milestones, metrics, and outcomes to drive improvement.
• Build partnerships with healthcare providers, public health agencies, and community organizations.
