Job summary

Responsibilities

  • Supports operational improvements and vendor coordination for population health initiatives.
  • Performs day-to-day reconciliation of quality incentive payments and financial records.
  • Tracks and reports on population health performance and incentive program metrics.
  • Maintains relationships with stakeholders and provides education/resources for CAP quality programs.
  • Ensures compliance with regulations and privacy policies (HIPAA, OSHA, etc.).
  • Participates in committees and supports special projects as assigned.

The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.

Position Summary

Reporting to the Senior Director, Population Health, the Population Health Program Coordinator will provide administrative, financial, and operational support for the Population Health unit. This role assists with program coordination, reporting, and finance-related tasks such as reconciling checks, processing payments, and maintaining accurate records. The coordinator ensures smooth program operations that improve support organizational quality and care initiatives.

Responsibilities
Program Coordination
  • Supports Manager and/or Director on implementing operational improvements and support activities to meet population health performance objectives.
  • Vendor management and support to facilitate payment of incentive dollars to participating practices. Assist with overall vendor coordination and documentation.
  • Tracks compliance with CAP IPA Membership Guidelines and Requirements as it relates to quality and regulatory requirements.
  • Supports preparation and distribution of reports on incentive program performance to participating CAP IPA practices and providers.
  • Supports maintenance of CAP newsletter and other educational resources for the CAP quality programs.
  • Supports the Population Health team on quality and regulatory activities and related projects, including payer and internal data management support
  • Regularly attends and prepares meeting minutes for Population Health meetings.
  • Performs other tasks and special projects as required and assigned within scope of work, education, and training.
Finance
  • Performs day-to-day coordination and reconciliation of quality incentive payments.
  • Reconcile incoming checks and payments for Population Health programs, or initiatives. Track and process financial transactions, and invoices in compliance with organizational policies. Maintain accurate financial records and report to leadership for review.
Strategy
  • Monitors key performance indicators and under the director of management, implements performance improvement initiatives, as needed. Supports implementing operational improvements.
People
  • Works collaboratively both across the organization and with external stakeholders to achieve established performance targets.
  • Establishes and manages relationships with stakeholders. Ensures engagement and consistent communication on deliverables and related matters.
  • Promotes staff professionalism and performance with coaching, training and feedback. Mentors others in individual and team accountability, modeling behavior and demonstrating best practices/techniques.
Other
  • Keeps current on all organizational, regulatory compliance and patient privacy trainings policies (e.g. infection control, public safety, EH&S, HIPAA, OSHA, etc.) and successfully complete all required training.
  • Ensures compliance with a safe and secure work environment at all sites by following all OSHA, EH&S and public safety policies and guidelines.
  • Maintain patient privacy and confidentiality according to HIPAA requirements at all times.
  • Participate in committees and task forces as appropriate and when assigned by manager.
  • Perform other related duties and responsibilities as assigned/requested.

Minimum Qualifications
  • Bachelor's degree or equivalent of education and experience.
  • Minimum of 3 years of related experience.
  • An equivalent combination of education and experience may be considered.
  • Excellent organizational skills, time management and the ability to set priorities among multiple competing objectives, tasks, and initiatives are required.
  • Strong proficiency in problem assessment, resolution, and collaborative problem-solving complex, and interdisciplinary settings.
  • Ability to communicate effectively in both oral and written forms. This position requires the ability to interact positively, constructively, and effectively with professional staff, providers, and organizational stakeholders.
  • Ability to work independently, follow through, and handle multiple tasks simultaneously.
  • Ability to work collaboratively with a culturally diverse staff demonstrating tact and sensitivity in stressful situations.
  • Strong proficiency in Microsoft Office (Word & Excel) or similar software is required and an ability and willingness to learn new systems and programs.
  • Ability to handle sensitive information with complete confidentiality.
  • Must be a motivated individual with a positive mindset and exceptional work ethic.
  • Must successfully complete systems training requirements.
