Job summary

Trillium Health Resources is seeking a Population Health Nurse for a remote North Carolina-based role within a multi-county MCO serving individuals with mental health and substance use needs. The position offers standard hours and work-from-home options with competitive benefits.

The ideal candidate will hold an RN license in NC, one year of nursing experience, and a valid driver’s license, with willingness to travel within the catchment area.

Qualifications

• Associate’s degree in Nursing (Required).
• One (1) year of experience as a Registered Nurse (Required).
• Active North Carolina RN license (Required).
• Valid driver’s license (Required).
• Must reside within North Carolina and be able to travel within catchment (Required).
• Preferred: BH/MH/SU/IDD population experience; QM/UM data analytics; CM/CNM/CCM/ACM certifications (Preferred).

Qualifications:

Required:

• Associate’s degree in Nursing and one (1) year of experience as a Registered Nurse.
• Active North Carolina Licensure as a Registered Nurse (RN).
• Must have a valid driver’s license.
• Must reside within North Carolina.
• Must be able to travel within catchment as required.

Preferred:

• Experience working with BH/MH/SU/IDD population.
• Knowledge of QM and UM procedures, as well as experience in using data analytics for population health management.
• Certified Managed Care Nurse (CMCN), Certified Case Manager (CCM), Accredited Case Manager (ACM) or equivalent certifications in care management or nursing.
