Job summary

RiseMe is seeking a Population Health Coordinator to support hybrid community health, care coordination, and program implementation across the FPCSN network in Philadelphia. The role combines frontline health worker activities with program administration under the Population Health Manager’s guidance.

Initial focus includes Title V CYSHCN and FOCUS Grant initiatives, with opportunities to support additional grants, outreach projects, and data-driven quality improvements across sites.

Qualifications

• Associate degree or equivalent in health-related field required.
• Minimum 3 years in community health, care coordination, or related field.
• Experience coordinating multiple priorities and documenting accurately.
• Certified Community Health Worker certification required or within 6 months of hire.

Responsibilities

• Coordinate day-to-day implementation of Population Health grants and programs.
• Maintain program workplans, timelines, and documentation.
• Assist with translating grant requirements into workflows and outreach processes.
• Coordinate activities across sites and communicate with internal teams and partners.
• Track progress and escalate barriers or risks to manager.
