Job summary

MSU Health Care is seeking a Population Health Coordinator to support quality, population health, and value-based care initiatives in partnership with the MOSAIC CIN, ACO, and Henry Ford Health. You will work closely with providers, practice teams, and quality leadership to improve patient outcomes, close care gaps, and advance preventive care through data-driven outreach and care coordination.

Qualifications

• Two to three years of college in healthcare, science, or business with related coursework.
• One to three years of related experience in healthcare settings.
• Experience with electronic medical records and electronic scheduling systems.
• Experience in data analysis, reporting, and healthcare quality programs.
• Proficiency with spreadsheet, word processing, and database software.

Responsibilities

• Identify and address care gaps through patient outreach and population health reporting.
• Support quality initiatives for preventive screenings, chronic disease management, and immunizations.
• Use Epic, registries, dashboards, and performance reports to prioritize outreach.
• Assist patients in navigating care across primary, specialty, hospital, and community settings.
• Support transitions of care after ED visits, hospitalizations, and post-acute care.
• Collaborate with providers, staff, and quality teams to improve outcomes and engagement.
• Conduct Social Determinants of Health screening and connect patients with resources.
• Educate patients on preventive care, MyChart utilization, and available services.
• Support CIN/ACO/PCMH initiatives through data collection and performance improvement.
