Trillium Health Resources has a career opening for a Population Health Manager - Inpatient Discharge Planning Team to join our team! The Population Health Manager - Inpatient Discharge Planning Team is responsible for providing administrative oversight and clinical supervision to the Inpatient Discharge Planning Team, a cross functional team of staff who perform tasks as liaison between Trillium and the local community hospitals and providers. The Population Health Manager - Inpatient Discharge Planning Team will be available to perform clinical triage, lead on emergency or crisis calls/contacts or provide guidance and support when needed or requested.
