MLK Community Healthcare seeks a Population Health Specialist to close care gaps and improve outcomes. You will lead data analysis on patient compliance, ensure Cerner data integrity, and develop outreach strategies to enhance appointment adherence across multiple clinics.

You will collaborate with the Population Health Manager to conduct chart audits, monitor performance metrics, and drive quality improvement while maintaining privacy guidelines. Some travel may be required.

Qualificações

• Bachelor’s degree in a health-related field is required.
• At least one year of customer service experience in a healthcare setting.
• Fluent in English; Spanish preferred; valid CA driver’s license.

Responsabilidades

• Lead data analysis and monthly reporting on disease management and preventative health.
• Synchronize patient data with Cerner to ensure data integrity for care delivery.
• Identify patients with missed appointments and develop outreach to improve adherence.
• Oversee appointment scheduling, tracking, and reporting across clinics.
• Document interactions in Cerner and follow up on missed visits.
• Monitor patient experience surveys and provide quality improvement insights.
• Conduct chart audits to ensure compliance with documentation standards.
• Collaborate with Population Health Manager to address medical group needs.
• Maintain confidentiality and privacy per regulations.
• Travel as needed to support multidisciplinary teams across locations.
