Job summary

Avera Health is seeking a Population Health Coordinator to manage and coordinate initiatives identified through contract and program priorities, financial analysis, and patient care needs. You will join a multidisciplinary team focused on moving health forward for patients and the organization.

The role involves data collection and analysis of patient populations, coordinating outreach, and ensuring timely communication and scheduling within the Population Health and Payer Strategies teams.

Qualifications

• Associate's degree 1–3 years related experience in operations, administration or healthcare.
• Bachelor's degree in Business Administration or Health Administration preferred.
• Ability to communicate effectively with others, hear, understand and distinguish speech.

Responsibilities

• Review medical history and compile activity documentation.
• Review patient's medical chart including care management and services.
• Demonstrates competence in validation of clinical data from multiple sources.
• Provide exceptional, high-quality customer service to patients, staff, and physicians.
• Prepare and analyze data supporting value-based arrangements using reporting tools.
• Work with Population Health and Payer Strategies teams to evaluate data results and contribute to committees and decisions.
• Demonstrate strong negotiation and influencing skills.
• Plan, schedule and coordinate events such as meetings and patient outreach.

A Brief Overview

Responsible for managing and coordinating population health initiatives identified through contract and program priorities, financial analysis, and value-based patient care needs. This includes coordination and completion of assigned projects, including project definition, assignment of task responsibilities, setting deadlines, monitoring progress etc. The Coordinator is an integrated member of the Population Health and Payer Strategy teams, which exist to provide strategic and tactical decision support to stakeholders at all levels of the organization. The Coordinator is responsible for managing and coordinating program outreach to patients as well as data collection and analysis of our patient populations. They are responsible for the communication, scheduling, and review of targeted patients within a population to aid in program success.

What you will do

• Review medical history, compiles and substantiates activity through documentation.
• Review patient's medical chart, including care management services, care coordination services, medical history, examination results, diagnostic tests, and other services for a subset of patients.
• Demonstrates competence and is proficient in validation of clinical data derived from a variety of data sources.
• Promotes and displays a positive customer oriented demeanor to patients, staff, and physicians to provide exceptional, high-quality customer service.
• Prepare and analyze data that supports value-based arrangements using tools and other reporting software to assist in data analysis and understanding.
• Working with the Population Health and Payer Strategies teams, evaluates data results and provides input to various committees/leaders, stakeholders, sponsors and team members at all levels.
• Demonstrate strong negotiation and influencing skills.
• Plan, schedule and coordinate events such as meetings, patient outreach, etc.

Required Education, License/Certification, or Work Experience: Associate's 1-3 years Related experience and/or training in operations, administration or healthcare.
