Boston Medical Center is seeking a Population Health Clinical Pharmacy Specialist to enhance cardiometabolic health in the Mattapan community. The role involves collaborating with healthcare teams to manage chronic conditions, develop individualized treatment plans, and provide patient education. Part of BMCHS Population Health, a new initiative aims to improve cardiometabolic health in the Mattapan neighborhood through a multidisciplinary clinical model. The Clinical Pharmacy Specialist works with patients at high risk of or with cardiometabolic conditions like diabetes, hypertension, and obesity. They collaborate with team members to address medication needs, optimize therapy, and support adherence, under the supervision of the program’s medical director. This role offers a unique opportunity to serve the Mattapan community and collaborate with national health experts.

ESSENTIAL RESPONSIBILITIES / DUTIES:
JOB REQUIREMENTS
Education: Degree in Pharmacy from an accredited college.
Certificates, Licenses, Registrations: Valid Massachusetts pharmacist license; Board Certification in Pharmacotherapy (BCPS), Ambulatory Care (BCACP), or Geriatric (BCGP), or obtain within one year.
Experience: Completion of PGY1 residency, ambulatory focus preferred. Experience in direct patient care and managing primary care chronic diseases is preferred.
Knowledge and Skills: Proficiency in pharmacy procedures, systems, and equipment; strong analytical, organizational, and interpersonal skills; excellent communication; ability to work with diverse populations; motivational interviewing skills; problem-solving ability; adaptability to healthcare changes; knowledge of healthcare systems; time management; proficiency in MS Office.
