Job summary

The Stanford Children’s Health | Lucile Packard Children’s Hospital Stanford is seeking a program colleague to support the Medical Home Program. You will provide outreach, training, and technical assistance to pediatric providers in private practice and clinic sites serving low-income counties within Alameda County.

The role collaborates with county public health and child services to promote developmental screening and timely referrals.

Qualifications

• Bachelor's degree in a work-related discipline/field from an accredited college or university.
• Two (2) years of progressively responsible and directly related work experience; experience with child health and/or development and/or pediatric primary care settings is preferred.

Responsibilities

• Maintains appropriate contacts and working partnerships with County Public Health, pediatric primary care providers and clinics, and public/private agencies to support medical homes, developmental screening, and appropriate referral and follow-up for children and youth.
• Identifies and outreaches to pediatric primary care practices and clinics in designated County that serve a high volume of low-income children.
• Trains providers and office staff on medical home concepts, developmental screening, and referral pathways for children and their families.
• Identifies systems barriers to provision of medical homes and developmental screening and identifies potential solutions.
• Conducts regular site visits to practice sites to provide technical assistance, additional training, and consultation for pediatric providers and staff on medical home concepts, developmental screening, and referral pathways.
• Assists in design, revision and distribution of medical home materials and other educational materials for pediatric providers, including updates on program, systems, and policy changes that affect pediatric practice and services to children and families.
• Participates in the collection, consolidation and assessment of program data for the purpose of monitoring the results and overall effectiveness. May recommend areas of improvement.
• Prepares project reports, policies, and procedures and disseminates them to senior management for project reports.

Minimum Qualifications

• Education: Bachelor's degree in a work‑related discipline/field from an accredited college or university.
• Experience: Two (2) years of progressively responsible and directly related work experience; experience with child health and/or development and/or pediatric primary care settings is preferred.
• License/Certification: None required.

Pay Range

Compensation is based on the level and requirements of the role. Salary within our ranges may also be determined by education, experience, knowledge, skills, location and abilities as required by the role, as well as internal equity and alignment with market data. Typically, new team members join at the minimum to mid salary range. Minimum to Midpoint Range (Hourly): $37.62 to $48.91
