Mohawk Valley Health System is seeking a Licensed Practical Nurse for the Specialty Office to deliver general nursing care in support of patient-centered care. The LPN will collaborate with physicians, RNs and a multi-disciplinary team to support health coaching, case finding and supportive care.

Responsibilities include gathering patient data, preparing for examinations, and assisting with procedures under clinical direction. NYS LPN licensure and BLS within 6 weeks are required.

Formación

• Graduate of a practical nursing education program.
• Current New York State licensure as an LPN.
• BLS certification within 6 weeks of hire date.

Responsabilidades

• Gather patient measurements and collect patient data.
• Record and report patient findings.
• Prepare patient for examination and treatment.
• Assist specialty providers with examinations, treatments and procedures.
• Perform tasks within the framework of supportive care under the direction of a licensed health care provider.
• Support self-care and the established plan of care.
• Utilize standard precautions at all times.
• Maintain equipment, supplies, inventory and a safe, clean work environment.
• Engage in quality improvement initiatives and continue education.
• Maintain cooperative communication within the healthcare team.
