Job summary

Revere Health seeks a Psychiatric Mental Health Nurse Practitioner (PMHNP) to join an integrated primary care and behavioral health team in Utah. You will deliver psychiatric evaluations and medication management within the primary care setting.

You will collaborate closely with PCPs, care managers, social workers, and clinicians to ensure accessible, coordinated mental health care and contribute to value-based care goals in a fast-paced clinic.

Qualifications

• Master's or Doctorate in Nursing with PMHNP certification.
• Active Utah APRN license (or ability to obtain before start date).
• DEA license with prescribing authority.
• Experience or strong interest in working as part of a primary care and behavioral health team.

Responsibilities

• Provide psychiatric evaluations for patients referred by the primary care team.
• Deliver medication management for common mental health conditions in primary care.
• Collaborate with primary care providers and behavioral health staff in joint visits and care planning.
• Use PHQ-9, GAD-7, ADHD scales to monitor outcomes and guide diagnosis.
• Coordinate referrals and transitions of care as needed.
