Responsibilities

• Conduct Medication Management appointments for new and established clients.
• Prescribe medications within scope of practice.
• Communicate with PCPs as needed and coordinate care.
• Maintain accurate patient records and complete progress notes.
• Complete paperwork and submit invoices in a timely manner.
