Aliya Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled nursing facility team in Palos Heights, IL. You will provide total nursing care to residents, participate in care plans, administer medications, and supervise team members to ensure high-quality resident care.

Qualifications

• Graduation from an accredited nursing education program.
• Active LPN license in Illinois.
• No prior abuse convictions.
• Strong communication and teamwork skills.
• Ability to follow policies, maintain safety, and deliver empathetic care.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as an LPN.
• Participate in developing and implementing individualized care plans alongside the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness regularly.
• Educate residents and families on care plans and medical procedures.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
