Job summary

The Alden Network in Illinois is seeking a Charge Nurse/ Nursing Supervisor to lead the PM shift in a skilled nursing facility. You will supervise RNs, LPNs, and CNAs, ensure safe, timely care, and coordinate with physicians and families.

The ideal candidate has an Illinois RN license and 1–2 years of experience in long-term care, plus prior supervisory experience. You will oversee regulatory compliance, document in medical records, and help with staffing coverage to maintain continuity of care.

Qualifications

• Current Illinois RN license required.
• Minimum of 1–2 years of experience in a skilled nursing facility or long-term care setting.
• Previous supervisory or charge nurse experience required.

Responsibilities

• Supervise and support nursing staff (RNs, LPNs, CNAs) during the PM shift
• Ensure residents receive safe, appropriate, and timely care
• Monitor and enforce compliance with all regulatory requirements (IDPH, CMS)
• Respond to and manage changes in resident condition, incidents, and emergencies
• Conduct rounds to assess resident care, staff performance, and unit operations
• Provide clinical guidance and support to staff as needed
• Communicate effectively with physicians, families, and interdisciplinary team members
• Ensure accurate and timely documentation in the medical record
• Follow up on pending issues from previous shifts and ensure continuity of care
• Participate in staffing coordination and assist with coverage as needed
• Address and document employee performance concerns and escalation when appropriate
