Job summary

UPMC Central PA is actively seeking full-time Adult and Senior Care Physicians for our Post-Acute Care team in Hanover. You will provide direct patient care across skilled nursing facilities and clinics, collaborating with discharge teams and care management to drive high-quality outcomes.

The role encompasses geriatrics, internal medicine, and primary care within a supportive system that values work-life balance and ongoing professional development.

Qualifications

• Board-certified or board-eligible in Primary Care, Internal Medicine, or Geriatric Medicine.
• Experience caring for geriatric populations across skilled nursing facilities and clinics.
• Ability to collaborate with facility leadership, care management, and other specialties.
• Willingness to supervise Advanced Practice Providers.

Responsibilities

• Provide direct patient care across multiple facilities in the aging continuum.
• Interface with discharge teams to optimize quality and safety.
• Collaborate with APPs and other physicians to coordinate care.
• Utilize EPIC EMR and diagnostic tools for patient management.

About the Position

• Provides direct patient care in multiple facilities across the aging continuum to include skilled nursing, rehab, personal care/assisted living, independent living and clinic.
• Interacts with facility leadership, hospital discharge team, care management and other specialties to drive optimal quality care with aligned goals.
• Post-Acute care offers a wide variety of healthcare fields, from general primary care and internal medicine to geriatrics and acute rehab services. Our clinician's crossover and work with several specialists, increasing their knowledge base and skillsets.
• Collaborates with and supervises Advanced Practice Providers.
• EPIC EMR, online diagnostics including PACS imaging and laboratory results.
• Clinical rounding M-F, attractive daily schedules with flexible models. Overnight and weekend call is all phone coverage, shared equally with partners and APPs, ratios very favorable.
