Job summary

UPMC Central PA is seeking full-time Adult and Senior Care Physicians to join our Post-Acute Care team in the Hanover region. You will provide direct patient care across skilled nursing facilities and clinics, focusing on geriatric medicine.

Qualifications

• Board-certified/board-eligible in Primary Care, Internal Medicine, Family Medicine, or Geriatric Medicine.
• Experience collaborating with facility leadership and care teams is valued.
• Experience with EPIC EMR and interfacility coordination is preferred.

Responsibilities

• Provide direct patient care across multiple facilities including skilled nursing, rehab, and clinics.
• Collaborate with facility leadership, discharge teams and care management to ensure high-quality care.
• Supervise Advanced Practice Providers and coordinate with other specialties.
• Utilize EPIC EMR and PACS for ordering and diagnostics.
• Support flexible scheduling with balanced call coverage.

About the Position:

• Provides direct patient care in multiple facilities across the aging continuum to include skilled nursing, rehab, personal care/assisted living, independent living and clinic.


• Interacts with facility leadership, hospital discharge team, care management and other specialties to drive optimal quality care with aligned goals.


• Post-Acute care offers a wide variety of healthcare fields, from general primary care and internal medicine to geriatrics and acute rehab services. Our clinician's crossover and work with several specialists, increasing their knowledge base and skillsets.


• Collaborates with and supervises Advanced Practice Providers.


• EPIC EMR, online diagnostics including PACS imaging and laboratory results.


• Clinical rounding M-F, attractive daily schedules with flexible models. Overnight and weekend call is all phone coverage, shared equally with partners and APPs, ratios very favorable.
