BJC Medical Group is seeking a Board Certified/Eligible Internal Medicine or Geriatrics physician to join West County Medical Associates in a post-acute care role serving the West St. Louis County region.

Qualifications

• MD or DO from an accredited medical school.
• Board Certified or Board Eligible in Internal Medicine and/or Geriatrics.

Responsibilities

• Provide admission, follow-up, and transitional care services in post-acute and long-term care settings.
• Manage acute and chronic medical conditions in adult and geriatric populations.
• Coordinate care transitions from Barnes-Jewish West County Hospital and other facilities.
• Collaborate with advanced practice providers, nursing staff, therapists, social workers, and specialists.
• Participate in interdisciplinary care conferences and family meetings.
• Conduct goals-of-care and advance care planning discussions.
• Drive quality initiatives to reduce avoidable hospitalizations and improve outcomes.
• Complete accurate EMR documentation.
• Foster relationships with facility partners and referral sources.

Practice Overview

The Post-Acute Care Physician will provide comprehensive medical care within skilled nursing facilities, rehabilitation centers, assisted living communities, and long-term care facilities. Working alongside an interdisciplinary care team, the physician will focus on improving quality outcomes, reducing readmissions, and ensuring seamless transitions of care.

Preferred

• Fellowship training in Geriatric Medicine.
• Experience in Post-Acute Care, Skilled Nursing, Long-Term Care, Hospital Medicine, or Transitional Care.
• Interest in value-based care and population health management.
