BJC HealthCare, in partnership with BJC Medical Group and West County Medical Associates, seeks a Board Certified/Board Eligible Internal Medicine or Geriatrics physician for a post-acute care role across skilled nursing, rehab, and long-term care settings in the St. Louis area.

The position offers academic affiliation with Washington University School of Medicine, collaboration across care continuum, and opportunities to impact transitions from hospital to post-acute settings.

Qualifications

• MD or DO degree from an accredited medical school.
• Board certification or eligibility in Internal Medicine or Geriatric Medicine.
• Missouri licensure eligibility and DEA certification.

Responsibilities

• Provide admission, follow-up, and transitional care in post-acute and long-term settings.
• Manage acute and chronic conditions in adult and geriatric patients.
• Coordinate transitions from hospital to post-acute facilities with care teams.
• Collaborate with APPs, nursing, therapists, social workers and specialists.
• Participate in interdisciplinary care conferences and family meetings.
• Conduct goals-of-care and advance care planning discussions.
• Lead quality initiatives to reduce readmissions and improve outcomes.
• Complete accurate and timely EMR documentation.
• Foster relationships with facilities and referral sources.

BJC Medical Group is seeking a Board Certified/Board Eligible Internal Medicine or Geriatrics Physician to join West County Medical Associates in a Post-Acute Care role serving patients throughout the West St. Louis County region.

Practice Overview

The Post-Acute Care Physician will provide comprehensive medical care within skilled nursing facilities, rehabilitation centers, assisted living communities, and long-term care facilities. Working alongside an interdisciplinary care team, the physician will focus on improving quality outcomes, reducing readmissions, and ensuring seamless transitions of care.

Key Responsibilities

• Provide admission, follow-up, and transitional care services in post-acute and long-term care settings.
• Manage acute and chronic medical conditions in adult and geriatric populations.
• Coordinate care transitions from Barnes-Jewish West County Hospital, Barnes-Jewish Hospital, and other referring facilities.
• Collaborate with advanced practice providers, nursing staff, therapists, social workers, and specialists.
• Participate in interdisciplinary care conferences and family meetings.
• Conduct goals-of-care and advance care planning discussions.
• Drive quality initiatives focused on reducing avoidable hospitalizations and improving patient outcomes.
• Complete accurate and timely EMR documentation.
• Foster strong relationships with facility partners and referral sources across the community.

Qualifications

• MD or DO from an accredited medical school.
• Board Certified or Board Eligible in:
  • Internal Medicine
  • Geriatric Medicine
• Eligible for Missouri medical licensure.
• DEA certification.
• Commitment to patient-centered, team-based care.

Preferred

• Fellowship training in Geriatric Medicine.
• Experience in Post-Acute Care, Skilled Nursing, Long-Term Care, Hospital Medicine, or Transitional Care.
• Interest in value-based care and population health management.
