Job summary

Hartford HealthCare in Thomaston, Connecticut is seeking a Per Diem, Adult (18+) Primary Care physician to join our outpatient clinic. You will deliver ambulatory care to adult patients with EPIC EMR and Ambient AI integration, supporting a fully integrated care model with Behavioral Health and Care Management.

The role offers weekday coverage with no weekend shifts, a flexible start and end times, and a supportive practice environment designed for physician success.

Qualifications

• MD or DO required for patient care.
• Board Certified or Board Eligible in Internal Medicine or Family Medicine.
• Unrestricted CT medical license or license eligibility.

Responsibilities

• Provide outpatient primary care to adults 18+ in the Thomaston clinic.
• Coordinate care within an integrated care model with Behavioral Health and Care Management.
• Collaborate with EPIC EMR and other clinic staff to ensure efficient workflows.
