Provide the full scope of services which fall under provider’s field of training, including but not limited to diagnosis, treatment, coordination of care, preventive care, and electronic health record (EHR) maintenance, while following guidelines contained within the quality management, risk management, infection control, customer service and safety programs for Access Community Health Network.

Clinical and Operational Responsibilities

General Clinical

  • Obtains health history and performs physical examinations.
  • Diagnoses, treats, counsels, and coordinates care for each patient.
  • Reviews and updates problem list and longitudinal plan of care at every visit.
  • Meets documentation standards and completes all progress notes within 24 hours following the patient visit.
  • Fulfills on-call responsibilities in accordance with the schedule developed by ACCESS and responds to calls within 20 minutes.
  • Prescribes medication in accordance with Illinois statute and professional practice guidelines.
  • Performs all duties in conformity with the quality standards and clinical guidelines established by ACCESS.
  • Meets Meaningful Use standards by using certified EHR technology to improve quality, safety, efficiency, and reduce health disparities, while engaging patients and families, improving care coordination, and maintaining privacy and security.
  • Manages EHR In-Basket, transfers responsibilities when appropriate, and responds to messages (lab results, refills, patient calls, etc.) within 72 hours.

Patient Support and Education

  • Provides culturally competent care.
  • Recognizes patients’ families as part of the care team.
  • Ensures patient and family needs are met and clinic policies are followed.
  • Encourages questions and concerns from patients and families.
  • Works with patients and families to promote self-management of care.
  • Conducts patient education, including pre/post instructions and reinforcement of follow-up care.
  • Utilizes printed educational materials and tailors the presentation to the patient's understanding.
  • Coordinates with care team and case management for comprehensive services.
  • Transfers patients with greater acuity appropriately.
  • Meets severity of illness admission criteria.

PCMH Requirements

  • Coordinates with health center staff to meet patient needs.
  • Participates in huddles and meets productivity standards.
  • Ensures timely and appropriate patient care with team involvement.
  • Ensures clinical tasks follow policies and procedures.

Provider Development

  • Participates in staff orientation and training.
  • Ensures care team meets certification requirements.
  • Participates in proficiency testing to maintain standards.
  • May serve as a preceptor for training programs.
  • Adheres to HRSA and FTCA regulations.
  • Performs other duties as assigned.

Requirements/Preferences

  • Doctor Osteopathy (D.O.) or Doctor of Medicine (M.D.) required.
  • Completed residency and Board Certification in primary care required.
  • Minimum two (2) years of outpatient primary care experience preferred.
  • Current Illinois licensure, DEA, and CME compliance required.
  • Knowledge of quality assurance, safety, infection control required.
  • EPIC EHR competency preferred.
  • Cultural competency and interpersonal skills essential.
  • Strong organizational skills required.

Working Conditions/Equipment

  • Ability to concentrate and communicate amidst interruptions.
  • Ability to stand, sit, and walk for up to eight hours.
  • Ability to lean and stretch during examinations.
  • Computer/laptop use required.
