Job summary

Legacy Care in Richmond, VA is seeking a compassionate physician to join our team on a part-time basis. You will provide medical care across post-acute, long-term care and assisted living settings, collaborating with interdisciplinary teams and supporting families.

You will conduct comprehensive assessments, develop treatment plans, and ensure timely documentation in the EMR while promoting continuity of care and quality outcomes.

Qualifications

  • MD or DO with board eligibility in Family Medicine, Internal Medicine or Geriatrics preferred.
  • Virginia medical license (unrestricted) and DEA registration.
  • Experience in post-acute, long-term care or geriatric settings preferred.
  • Strong clinical assessment, documentation and teamwork skills.

  • Perform medical evaluations and treatment in post-acute and long-term care settings.
  • Conduct initial, routine and follow-up assessments with interdisciplinary teams.
  • Develop and revise individualized treatment plans with patients and families.

Role Overview: Richmond, VA

We are seeking a skilled and compassionate physician to join our Richmond team on a part-time basis. This role offers the opportunity to provide high-quality, person-centered medical care within post-acute, long-term care and assisted living settings. The successful clinician will work collaboratively with interdisciplinary teams, support patients and families, and promote continuity of care through timely assessment and treatment.

Key Responsibilities
  • Provide comprehensive medical evaluations, diagnoses and treatment for patients in post-acute, long-term care and assisted living settings
  • Conduct initial, routine and follow-up assessments, including review of medical history, medications, symptoms and current health concerns
  • Develop, implement and revise individualized treatment plans in collaboration with patients, families, facility staff and interdisciplinary teams
  • Manage acute and chronic medical conditions and respond appropriately to changes in patients’ clinical status
  • Order, interpret and follow up on diagnostic tests, laboratory results and other clinical investigationsPrescribe and manage medications safely and effectively, including monitoring for treatment response, adverse effects and potential interactions
  • Provide appropriate evaluation and management of urgent medical concerns within the scope of practice
  • Participate in care planning meetings, interdisciplinary reviews and family consultations as required
  • Coordinate care with nursing staff, primary care clinicians, specialists, hospitals, rehabilitation providers and community resources
  • Provide education and support to patients, families and facility staff regarding diagnoses, treatment plans, medications and preventive care
  • Maintain accurate, timely and clinically appropriate documentation in the Electronic Medical Record (EMR)
  • Contribute to quality improvement initiatives and promote safe, culturally sensitive and person-centered care
Qualifications And Required Skills
  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school
  • Current, unrestricted Virginia medical license and eligibility to practice in Virginia
  • Board certification or board eligibility in Family Medicine, Internal Medicine, Geriatric Medicine or a related specialty preferred
  • Current DEA registration and prescribing authority, as required
  • Experience in post-acute, long-term care, assisted living, geriatric, hospital or related clinical settings preferred
  • Strong clinical assessment, diagnostic reasoning and treatment planning skills
  • Proficiency with Electronic Medical Record systems and timely, accurate documentation practices
  • Excellent communication, collaboration and problem-solving abilities
  • Compassionate, culturally sensitive and person-centered approach to patient care
  • Ability to work independently while collaborating effectively with facility staff and multidisciplinary care teams
  • Availability to provide reliable part-time coverage based on patient and facility needs
