Burlington Health & Rehabilitation Center is seeking a dedicated Physician or Nurse Practitioner to join our Nursing Services team and support the delivery of high-quality care in a skilled nursing and long-term care setting. Qualifications

• Active and unrestricted license as a Physician or Nurse Practitioner in the practicing state.
• Minimum of three years of professional experience.
• Experience in a nursing home, skilled nursing facility, hospital, or related healthcare setting preferred.
• Knowledge of medical and nursing practices, procedures, and regulations related to long-term care.
• Strong clinical assessment, documentation, communication, and leadership skills.
• Ability to work collaboratively with the Director of Nursing, Administrator, Medical Director, and interdisciplinary team.
• Current CPR certification preferred or as required by facility policy.
• Prior geriatric, skilled nursing, or long-term care experience is a plus.

Responsibilities

• Provide clinical evaluation, treatment, and follow-up care for residents.
• Conduct rounds to observe residents, assess clinical needs, and ensure care plans are being followed.
• Manage acute and chronic conditions in collaboration with the interdisciplinary care team.
• Participate in admission decisions and resident status reviews as needed.
• Collaborate with nursing leadership, physicians, consultants, department heads, and community agencies to coordinate care and resolve clinical concerns.
• Communicate with residents, family members, medical staff, nursing staff, and interdisciplinary team members regarding care needs and changes in condition.
• Support the development, implementation, and evaluation of resident care plans.
• Assist with identifying and responding to resident incidents, unusual occurrences, and changes in condition.
• Participate in QAPI, facility assessments, performance improvement initiatives, and care-related committees as needed.
• Support infection-control practices in accordance with current guidelines.
• Assist with survey preparation, mock surveys, plan-of-correction follow-up, and regulatory compliance efforts.
• Promote safe work practices, teamwork, mutual respect, and effective communication.
• Provide direct clinical care as necessary.
• Support facility leadership in maintaining quality standards and improving resident outcomes.
