Job summary

Burlington Health & Rehabilitation Center in Burlington, VT is seeking a Physician or Nurse Practitioner to join our Nursing Services team in a part-time role. You will provide direct clinical care, perform rounds, and guide resident assessments and care planning in a skilled nursing setting.

The ideal candidate will have an active license in the state, experience in long-term care, and strong skills in clinical assessment, documentation, and communication.

Qualifications

  • Active and unrestricted license as a Physician or Nurse Practitioner in the practicing state.
  • Experience in a nursing home/long-term care setting preferred.
  • Strong clinical assessment, documentation, communication, and leadership skills.

Responsibilities

  • Provide clinical evaluation, treatment, and follow-up care for residents.
  • Perform rounds to observe residents and ensure care plans are followed.
  • Manage acute and chronic conditions with the interdisciplinary care team.
  • Participate in admission decisions and resident status reviews as needed.
  • Collaborate with nursing leadership, physicians, consultants, department heads, and community agencies to coordinate care and resolve clinical concerns.
  • Communicate with residents, family members, medical staff, nursing staff, and interdisciplinary team regarding care needs, changes in condition, and concerns.
  • Support the development, implementation, and evaluation of resident care plans.
  • Assist with identifying and responding to resident incidents, unusual occurrences, and changes in condition.
  • Participate in QAPI, facility assessments, performance improvement initiatives, and care-related committees as needed.
  • Support infection control practices in accordance with current guidelines.
  • Assist with survey preparation, mock surveys, plan of correction follow-up, and regulatory compliance efforts as needed.
  • Promote safe work practices, teamwork, mutual respect, and effective communication.
  • Provide direct clinical care as necessary.
  • Support facility leadership in maintaining quality standards and improving overall resident outcomes.

We are seeking a dedicated Physician or Nurse Practitioner to join our Nursing Services team and support the delivery of high-quality care in a skilled nursing / long-term care setting.

As a key clinical provider, you will assist with resident assessments, care planning, medical oversight, and coordination of services. You will help guide care decisions, support nursing staff, communicate with families, and promote a resident-centered approach focused on safety, quality, dignity, and positive outcomes.

Key Responsibilities:

  • Experience in a nursing home, skilled nursing facility, hospital, or related healthcare setting preferred.
  • Knowledge of medical and nursing practices, procedures, and regulations related to long-term care.
  • Strong clinical assessment, documentation, communication, and leadership skills.
  • Ability to work collaboratively with the Director of Nursing, Administrator, Medical Director, and interdisciplinary team.
  • Current CPR certification preferred or as required by facility policy.
  • Prior geriatric, skilled nursing, or long-term care experience is a plus.
