Job summary

Hill Country Community Clinic in California seeks a compassionate Physician to join our team. The role focuses on patient-centered primary care for individuals of all ages, ensuring comprehensive, integrated healthcare in a community health setting.

Responsibilities include direct patient care, preventive services, coordinating with a multidisciplinary team, maintaining accurate EHRs, and supporting outreach and health education aligned with FQHC guidelines. California license required.

Qualifications

• MD or DO degree from an accredited medical school.
• Board certification or eligibility in Family Medicine, Internal Medicine, or Pediatrics.
• Active and unrestricted California medical license.

Responsibilities

• Provide direct patient care, including preventive care, diagnosis, treatment, and management of acute and chronic conditions.
• Develop and implement individualized treatment plans and educate patients on health maintenance and disease prevention.
• Collaborate with nurses, behavioral health providers, case managers, and care coordinators to coordinate patient care.
• Maintain accurate and up-to-date electronic health records (EHR) in compliance with federal, state, and organizational standards.
• Participate in quality improvement initiatives, peer reviews, and compliance with FQHC guidelines and best practices.
• Provide culturally competent care tailored to the needs of a diverse patient population.
• Supervise and mentor advanced practice providers as needed.
• Support community outreach, health education, and patient engagement initiatives.
• Adhere to all FQHC regulatory requirements, including HRSA, HIPAA, and OSHA guidelines.
