Job summary

Signify Health, part of CVS Health, seeks physician contractors (MD/DO) to conduct in‑home health evaluations for Medicare Advantage, Medicaid, and select plans. You will perform visits, review medical history, and assess overall health in the patient’s home.

This is an independent contractor role with flexible scheduling, centered on pre‑scheduled home visits. Licensure in the relevant state is required or willingness to enroll, with potential for eligibility‑based travel considerations.

We’re hiring physician (MD/DO) contractors

To conduct In‑home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

• Conduct in‑home visits that focus on a person’s overall health and well‑being and identify potential barriers to care.
• Earn up to $3,000 to $5,000 each week (average 6‑9 visits daily, base rate of $100 per completed assessment).
• Increase your per‑visit rate up to 135% with our Market Multiplier.
• Medicaid license or willingness to enroll (with our support) strongly preferred.
• Earn additional compensation via our pay‑per‑test model for visits where the health plan member qualifies to receive additional diagnostic and preventive tests.
• Shifts available Monday – Sunday (no nights/weekends required).
• Short‑Term or Long‑Term basis.
• Pre‑scheduled visit in‑home with health plan members.
• Airfare, hotel, and rental car available (depending on eligibility).
• Malpractice coverage.

These visits include a medication and medical history review, as well as a physical evaluation performed in a health plan member’s home. Physician contractors can earn a competitive wage (or supplement their current income) while managing other personal and clinical commitments. A pre‑populated, patient‑specific form is provided for each evaluation, and the physician does not prescribe medicine, order lab tests, or alter people’s current treatment regimen.

This is an excellent opportunity for resident physicians to earn additional income or for more seasoned physicians seeking a change of pace.
