Job summary

Johns Hopkins Health System is seeking a Physician Liaison/Record Integrity Analyst who performs detailed qualitative and quantitative analysis of hospital medical records to ensure completion and compliance with policy and state and federal regulations, enabling timely documentation for coding and billing.

Under HIM supervision, you will conduct clinical pertinence audits against Medical Staff Rules, The Joint Commission Standards, and CMS regulations, reporting results to Medical Staff

Qualifications

• Associates Degree or active enrollment in an accredited Health Information Management Program.
• RHIT/RHIA credential with one-year Health Information Management experience; RHIT preferred.

Responsibilities

• Review, identify and assign documentation deficiencies to the correct provider(s).
• Continually monitor hospital record completion and ambulatory encounter compliance.
• Serve as a resource to physicians and other providers regarding timely and complete documentation for coding, TJC and CMS purposes.
• Serve as the primary contact person for providers investigating record completion issues.
• Collaborate with hospital departments to resolve documentation issues.
• Communicate with providers regarding incomplete documentation and elevate to physician and hospital leadership according to policy.
• Implement physician suspension procedures when records are not completed within the required time frame.
• Reconcile deficient clinical documentation reports utilizing excel spreadsheets.
• Identify incorrect note types and perform note type chart corrections.
• Review and manage the verbal/co-sign orders according to hospital policy.
• Locate, track and route problematic provider dictations to the correct patient record within the EHR.

Education
