Job summary

HarmonyCares, a national value-based home-based primary care provider, seeks a Physician Leader to oversee a pod of 4-5 APPs with a 140-patient panel. The role is full-time, Monday–Friday, with no weekends or evenings, balancing admin/clinical oversight with hands-on patient care.

The pod leader will drive quality outcomes, collaborate with the clinical team, and participate in performance improvement within a physician-led model, with strong bonus potential and comprehensive benefits.

Qualifications

• Must be a Doctor of Medicine or Osteopathy and maintain active medical licensure.
• Board certification preferred.
• Strong leadership and communication skills required.
• Experience with value-based care is a plus.

Responsibilities

• Provide clinical leadership and oversight of the pod team.
• Collaborate with APPs and other clinical staff on patient care.
• Lead quality and safety initiatives and monitor performance metrics.
• Ensure accurate documentation and diagnostic accuracy.
• Oversee care plans and high-risk patient management.
