As a remote Primary Care physician, you will be part of a groundbreaking care model that integrates advanced AI from K Health to deliver smarter, faster, and more personalized care, connected within Hartford HealthCare. Responsibilities
  • Co-manage patients with their assigned primary care physician
  • Educate patients on appropriate treatments and care plans for their health needs
  • Manage patient follow-ups for chronic care programs
  • Prescribe and refill medication as appropriate
  • Drive high-quality care with a focus on patient outcomes
  • Elevate remote care by providing compassionate and meaningful patient encounters
  • Critical thinking to devise creative solutions to problems that arise or escalate as appropriate
  • Collaborate with Clinical Team Members to build clinical guidelines and policies
Qualifications

What we're looking for:

  • 3+ years of post-residency work experience
  • Must have an active CT state medical license, IMLC preferred
  • Board-Certified Family or Internal Medicine physician
  • Demonstrated experience in Acute Care or Urgent Care settings
  • Brick-and-mortar primary care and behavioral health experience, ability to treat both acute and chronic care conditions
  • Should be tech-savvy, proactive, organized, detail-oriented, and have telemedicine experience
  • Extensive experience handling 3+ cases per hour
  • Must have prior experience with EPIC EMR
  • Clean background and medical malpractice history
  • Willing to commit 40 hours per week, 36 clinical hours
  • Must be able to work Monday and Friday with flexibility over starting hours
  • Must be able to work one full weekend every month
