Qualifikationen

• MD or DO degree from an accredited medical school.
• Unrestricted Ohio medical license and current DEA registration.
• National Provider Identifier.
• Board certification or eligibility appropriate to the clinical role.
• Strong communication skills, sound clinical judgment, emotional intelligence, and a commitment to collaborative care.
• Commitment to evidence-informed practice, patient safety, education, and continuous improvement.
• Experience with Epic is preferred.

Aufgaben

• Build and maintain a patient-centered practice with comprehensive evaluation, treatment, and follow-up.
• Develop individualized care plans incorporating medical treatment, nutrition, activity, and behavior change.
• Collaborate with physicians, APPs, dietitians, health coaches, and others to coordinate care.
• Develop relationships with referring clinicians within Cleveland Clinic and the broader community.
• Participate in shared medical appointments, patient education, and virtual care as appropriate.
• Improve access, patient experience, clinical workflows, documentation, and quality of care.
• Maintain timely documentation and patient confidentiality per policies.
• Supervise and mentor APPs and other clinicians as assigned.
