Trinity Health IHA Medical Group in Brighton, MI is seeking a Physician in Geriatrics to provide comprehensive outpatient primary care for patients 65 and older, manage chronic conditions, and lead preventive care. Qualifications

• Board-certified or eligible in Family Medicine, Internal Medicine, or Med-Peds
• Minimum 5 years of post-residency clinical experience
• Outpatient primary care experience preferred

Responsibilities

• Provide comprehensive outpatient primary care patients 65 and older.
• Managing chronic conditions and developing personalized treatment plans
• Performing preventative care, wellness visits, and routine screenings
• Collaborating with nurse practitioners, specialists, and care teams to optimize patient outcomes
• Maintaining accurate and timely electronic health records documentation
• Participating in quality improvement initiatives to enhance patient care and clinic operations
• Staying up to date on evidence-based medical practices and advancements in primary care
• Engaging in community outreach and health education efforts when applicable

Duties & Responsibilities

A typical day may include:

• Provide comprehensive outpatient primary care patients 65 and older.
• Managing chronic conditions and developing personalized treatment plans
