Job Opportunity: Primary Care Physician in Chicago, IL

We are seeking a Board-Certified Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine Physician. Spanish speaking skills are a must. This is an outpatient-only, full-time, permanent position with the following details:

1. Schedule: Monday to Friday, no nights or weekends
2. Patient Panel: Small panels for consistency and focus, averaging 12-15 patients per day
3. Care Model: Value-based care with competitive bonuses rewarding outcomes
4. Role: Provide personalized primary care for older adults on Medicare, aiming to keep patients healthy and active
