Job description

Position: Outpatient Geriatric Medicine Physician

Seeking a Board-Certified FM, IM, or Geriatric Medicine Physician for a full-time, permanent outpatient position. Work schedule is Monday to Friday, with no nights or weekends, and small patient panels (12-15 patients per day) for personalized care.

Responsibilities include providing personalized primary care to older adults on Medicare, focusing on maintaining health and quality of life. The practice operates under a value-based care model with structured bonuses to reward outcomes.
