Job summary

Dartmouth-Hitchcock is seeking an Academic Geriatrician to provide geriatrics care at nursing homes and assisted living facilities, primarily in Lebanon, NH, and surrounding areas. The role includes collaborating with the General Internal Medicine team to ensure patient-centered care across various settings including independent living and skilled nursing.

Candidates must be Board Certified/Board Eligible in Internal Medicine or Family Medicine. Fellowship training in geriatrics is a plus, and medical licenses in NH and VT are required.

Qualifications

Job description

Overview

Academic Geriatrician sought by the General Internal Medicine section at Dartmouth-Hitchcock Medical Center to join the team and provide support at Nursing Home and Assisted Living facilities. Service locations include Hanover, NH; Lebanon and Claremont, NH; and Windsor, VT. The facilities offer a continuum of care, including independent living, assisted living, short-term rehabilitation, skilled nursing, memory care and Alzheimer’s care, respite care, and end-of-life care.

• Provide geriatrics care and support at nursing home and assisted living facilities within the service area.
• Collaborate with the General Internal Medicine team to deliver patient-centered care across the care continuum described above.
• Contribute to maintaining and developing an established patient base within the academic health system.
