Job summary

The Sonoma County Indian Health Project, Inc. seeks a Family Practice Physician to provide patient-centered care within a team-based medical home. You will work under the Chief Medical Officer, delivering primary care, ordering diagnostics, and coordinating with PAs and NPs across the clinic.

You will maintain high-quality notes in the EHR, participate in CQI, and engage in outreach when needed, while upholding cultural sensitivity and professional standards in a Native American community.

Qualifications

  • Knowledge of modern methods, theories, techniques and procedures used in medicine.
  • Knowledge of requirements used for Quality Assurance.
  • Knowledge of principles, techniques and best practices related to team based primary care models.
  • Knowledge of related accreditation and certification requirements, strong leadership skills.

Responsibilities

  • Take appropriate history, perform physical examination of patients, order relevant diagnostics and deliver appropriate therapy.
  • Referral for examination and consultation to other providers when problems are unfamiliar or outside scope.
  • Supervision of PAs and NPs according to standardized procedures defining their scope.
  • Participate in on-call duties and collaborate in case review/management panels.
  • Maintain legible, accurate notes in the Electronic Health Record in SOAP format.
  • Maintain problem lists, medication profiles, and health maintenance for all clinic patients.
  • May participate in outreach clinics and hospital credentialing processes.
  • Demonstrate professionalism, teamwork, and cultural sensitivity with Native American communities.

Job description

POSITION SUMMARY: The Family Practice Physician reports to the Chief Medical Officer. The purpose of this position is to perform duties of a medical physician and abide by the physician’s oath when providing care to SCIHP patients in a team based, collaborative, patient centered medical home.ESSENTIAL FUNCTIONS:Take appropriate history, perform physical examination of patients, order relevant diagnostics and deliver appropriate therapy according to standard medical practice.Referral for examination and consultation to other providers when problems are unfamiliar or outside scope of practice.Supervision of PA’s and NP’s according to the standardized procedures defining their scope of practice.Some training of support staff.Rotate on-call duties during the week and on weekends with other providers.Demonstrate competent knowledge in delivering health care to patients through peer review, participation in Quality Improvement Program, and collaboration with peers in case review/management panels.Assure quality and safety by adhering to PCMH principles, including teamwork, evidence-based medicine, CQI, and active participation of patients in decision making.Nursing home visits on agencies patients.Punctual participation in departmental, organizational meetings, and committees as assigned.Maintain legible, accurate, and timely completed notes on clinic patients in S.O.A.P. style in the Electronic Health Record.Maintain problem lists, medication profiles and health maintenance on all clinic patients.May participate in outreach clinics to Kashia or Point Arena.Respectful, timely and collegial communication with all staff.Demonstrate exemplary professionalism and cultural sensitivity.Maintain credentialing by keeping all required licenses current, including but not limited to, California Medical License, DEA License, CME hours, CPR Certifications and any other required licenses.EDUCATION AND EXPERIENCE:A current License by the Medical Board of the State of California.
