Position Overview:

1. Full-time position
2. 4 or 5-day work week available
3. Join a team of 5 physicians and 4 APPs
4. See patients in skilled nursing facilities and through home visits
5. Average of 8-10 patients per day
6. Use Epic Electronic Medical Record
7. Opportunities for leadership and participation in organizational decision-making
8. Influence practice policies and procedures
