Position Details:

1. Full-time, permanent outpatient-only role
2. Monday to Friday schedule, no nights or weekends
3. Small patient panels for personalized care (2-15 patients per day)
4. Value-based care model with structured bonuses rewarding outcomes
5. Provide primary care for older adults on Medicare, promoting health and quality of life
