Job description

Medical Director - In-Home Primary Care

The Medical Director will be responsible for leading, as a dyad partner with the Mobile Practice Manager, the in-home primary care provider teams in the specified geography.

The At Home division is an interdisciplinary team focusing on primary care and transitional care support for our highest-need patients in their homes.

The primary responsibilities for the Medical Director include direct management, oversight, and coaching of the local team. The Medical Director is part of the At Home division's leadership team, which helps develop programs such as complex care coordination, chronic disease management, and psychosocial support.

The Medical Director will see patients in their homes up to 3 days a week alongside provider colleagues. Additionally, this leader will be a key thought partner in providing feedback on our clinical model, especially regarding the At Home Program.

Qualifications
  • Motivated, experienced Primary Care Providers
  • Board Certified in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
  • Experience with managed care and/or value-based practice
  • A commitment to older adults and low-income patients
  • Mission-focused career aiming to impact change and improve health outcomes for Medicare patients
