Seeking an additional BE/BC Family Medicine physician to join our established practice southwest of Champaign, Illinois.

1. 100% outpatient; Monday - Friday opportunity with flexible scheduling
2. Patient Center Medical Home Model
3. Leads a collaborative team including physician, APP, nurse, and Certified Medical Assistant to provide patient care in an ambulatory setting
4. Call is 1:5 telephone only, supported by nurse advisory line
5. Qualified for RIMSAP/IDPH loan obligation
6. Opportunity to teach Family Medicine residents and medical students
7. Located 20 minutes from Champaign-Urbana, a diverse community of 195,000 residents
8. Centrally located between Chicago, St. Louis, and Indianapolis
