Job summary

Union Community Care in Pennsylvania seeks a Physician to join a care team delivering patient-centered, culturally sensitive primary medical care. You will assess patients, develop treatment plans, order tests and refer to specialists when needed, in line with the clinic’s mission of whole health.

The position requires an active Pennsylvania MD or DO license, board certification or eligibility in the specialty, and strong English communication.

Qualifications

  • Board Certification or Eligibility in the physician's specialty.
  • Proficient English language skills.

Responsibilities

  • Provide primary medical care services to Union patients in a team setting.
  • Refer patients to medical specialists when necessary.
  • Prescribe medications per state board authority.
  • Perform examinations and formulate treatment plans with appropriate tests and referrals.
  • Document care plans and ensure timely execution.
  • Counsel and educate patients or delegate education to care team.
  • Stay updated on clinical guidelines and best practices.
  • Complete required forms for billing and funding sources.

The Physician is responsible for the fulfillment of all obligations as noted by their professional association. (Most cases will be the ABFP).SPECIFIC JOB DUTIESWorks as part of a care team to provide patient-centered, culturally sensitive care, using evidence-based medicine, to Union patients.Provides primary medical care services to patients of Union in accordance with training, experience, certification, insurance and licensure in a manner consistent with guidelines established in the principles of practice at Union and its mission statement.Refer patients to medical specialists or other sources of service when necessary.Prescribes and/or dispenses required medication to patients according to the individual prescriptive authority granted by the Pennsylvania State Board.Performs examinations and formulates treatment plan of patients including appropriate history, physical examinations, appropriate surgical procedures, laboratory and x-ray testing, and referrals as necessary. Fully documents plan of care; and executes care in a timely manner.Fully documents plan of care; and executes care in a timely manner.Counsels and educates patients regarding their physical or mental health or delegates such education to an appropriate care team member.Stay abreast of changes in clinical guidelines, and best practices.Ensure timely completion of forms presented to him/her on behalf of the patient regarding the patient’s physical or mental health conditions; for the proper billing of patients and third-party payers; and forms necessary for other funding sources.Performs other work-related duties as assigned.POSITION REQUIREMENTSLicensed M.D. or D.O. in the Commonwealth of PennsylvaniaBoard Certification/Eligibility in their specialty.English proficiency requiredESSENTIAL FUNCTIONSIn order to fulfill the requirements of this position, duties 1-10 are considered essential functions of the job.ORGANIZATIONAL INVOLVEMENTThis position is required to participate in mandatory all staff meetings, team meetings and trainings.
